Autologous Fat Transplantation – A Paradigm Shift in Breast Reconstruction

چکیده
Fat grafting to the breast is not a new concept. It has been under consideration since the 19th century when pioneers attempted inserting bulk volumes of fat (Neuber, 1893, Czerny 1895). Interest in fat grafting was rekindled with the introduction of liposuction, which provided an easy and reliable source of fat. After a brief pause in evolution of breast fat grafting due to concerns over confounding radiologic findings, and risk of carcinogenesis, fat grafting is once again at the forefront of cosmetic and reconstructive breast surgery as these concerns are have become more manageable. The plastic surgery community has re-assessed its position and has called for further clinical and basic science research in this versatile technique. As discussed below, fat grafting is becoming an invaluable tool for breast reconstruction both for core reconstruction and as an ancillary technique after various procedures for breast reconstruction.

BACKGROUND Latissimus dorsi flap is one of the best options for immediate and delayed breast reconstruction. However, this technique is limited by the tissue volume provided by the flap. To improve breast volume while reducing complications, fat grafting is now very often used in addition to latissimus dorsi flap. To the best of our knowledge, fat grafting was always performed as a second-line ...

Autologous fat transfer (lipofilling) is becoming an invaluable tool for breast augmentation as well as for breast reconstruction. Autologous lipofilling has several advantages, including biocompatibility, versatility, natural appearance, and low donor site morbidity. The main limitation is unpredictable fat graft resorption, which ranges from 25% to 80%, probably as a result of ischaemia and l...
متن کاملAutologous Fat Grafting for Whole Breast Reconstruction
SUMMARY This is the first reported case of a patient who had a single-stage large-volume breast reconstruction with autologous fat grafting, following rotation flap approach (RoFA) mastectomy. The purpose of this case study was to evaluate the viability of reconstruction of the breast by autologous fat grafting alone, in the context of RoFA mastectomy. The hypothesis was that there would be min...
